I had some double flowers on my Naga Morich plant that turned into Siamese peppers, with one being substantially smaller, but attached to the other. The extra pepper growing out the top looked a lot like the formation growing out on the top of yours there.

Dunno what to tell you about the type of pepper, or the cross you've got there though.

Ive never seen it on peppers but I have seen tails on peaches.... Cool pepper

It almost looks like you're meant to 'open' the pepper using the flap....a bit like a ring-pull on a coke can!
Not exactly like yours, but one of my Macedonian plants has an offshoot going on..
