Find Mike Dorsey or Jimmy Shadow. They will design and tattoo whatever you want. Some of the best artists in the area.
With that being said, I have tattoos by both of them and they are very creative artists, but I have no idea where their shops are currently because I haven't had a tattoo done in quite some time. They are both in the greater Cincinnati area somewhere. Mike used to be on short vine, don't know if he still is.
