seeds Seed placement?

I was wondering the correct way to place the seed once sprouted in a paper towel. I will be using both pro-mix and rockwool cubes.
 
Every one I've taken from towel to dirt has died.

From these deaths, I've made some assumptions.

1. Dirt kills plants. Needs further investigation.
2. The seed cap goes up, not down, when putting in the dirt.
3. Cover the transplants with something to hold in moisture. They will die in roughly 3.8 seconds from dehydration in a cup that has a 1 cm top of dry soil.

Nothing more specific to your situation, but I assume others will chime in with something actually helpful.
 
I place them with the tap root down. Then after a day or two gently push the seed cap into the side wall of the hole to avoid helmets.
