Other than your noise that seems a bit higer (3-4 times my noise and I have a noisy line), everything seems fine. Is that a stable 640k ? I'm willing to bet they forgot to uncap your line from your old contract.

Indeed, check snr and attenuation too, I have a feeling it might be fluctuating. Check your indoor circuit, if it's old or so, check the wall mount plug etc.   If you have access to the wall mount plug or are able to modify the adsl cable to the modem, I would try to decouple RD+ or RD-...
