fermenting Vinegar woes

White fuzz suddenly covering my vinegar - wasn't there 2 days ago, appeared very suddenly.
What might cause this to come on so quickly?

Acidity too low

I recently added a large bottle of wine - cut it too much I guess?

Wouldn't that also be a problem in the initial stage though?
