Is this a caterpillar or a disease? Is the bug inside? Can I still eat this poor baby?

Looks too big (to my untrained eye) to be bug damage. I'm not aware of any caterpillars that eat the fruit directly.

I'd guess some sort of vertebrate creature, which means you have an excuse to sit in the bushes clutching a shotgun and muttering to yourself.
