labels labels for 5 oz woozy

I just ordered 12 dozen 5 oz woozy jars for my first go around with trying to package up some nice bottles. Where do you get the labels for these hot sauce bottles ? And any ideas for creating a label would help. I have a few ideas for a name. Thanks !

Thanks to Salsa Lady I turned to Avery #15664 labels - 6 on a page, fit a 5oz woozy perfectly and can be printed at home on my old broken down ink jet.  Note: I get much better results when I take them to the office supply store and get em printed on their laser printer.    
 
EDIT:  You can download the template for the above labels here.  Then you open ms word, bring up the template, insert your artwork and print (or save to flash drive and take to office max).  :)

i order my labels on a roll from Met Speed Label. great price, fast turnaround and a quality product.
 
3.25 x 5.5" label in gloss overlaminate, rounded corners help with alignment perception if its slightly off. 
 
my labels are 2 color which ran me $50 per plate for the press and a 'per label' cost of just under 9¢ each for 2000 in rolls of 400 iirc

I have been getting my waterproof vinyl inkjet labels at http://labelsbythesheet.com and they're 6-up on a sheet and measure 4" x 3.3". Perfect for my 5 oz. bottles I get from http://www.ebay.com/usr/glassbottleoutlet_com
