For the plants I'd recommend Refining Fire - 5/6 already, what's one more?  (hint: a good thing)  Juanitos might have some and send you smaller quantities.  sicman might do so as well.  ajdrew might be shipping plants too.  check the vendor section of the website.

For the rest; Blueberries are now in stock at a lot of home depot/walmart garden center, farm n fleet, local nursery places, etc.  They will be 1-2 year old bareroot plants that you'll take home, dig a hole, add ferts/compost and then plant.  I recommend protecting them w chicken wire as rabbits eat the shoots/bark.  Kinda finicky and I've had better luck w raspberries, gooseberry and blackberry.

Lettuce/Cukes/Spinach - all can be sown directly in the garden.  Lettuce and Spinach have already been planted at my house (zone 5a) and I've started some cukes inside so they'll be about 4 weeks old at plant out (mid-late May).  Seeds for many varieties of lettuce/spinach/cukes will be within short distance of the blueberry plants in the garden centers, but I've also seen them at grocery stores, etc.

Happy growing and best of luck for a great 2015 season!   :)
