Does anyone use / have a grow log or database?

Does anyone have a spreadsheet or database of any kind to document and track plantings and grow logs?  With charts, etc?
 
Germination dates, methods, transplant dates, medium type, fertilization dates, fert amounts, size, growth rates, pod maturity, etc?
 
Anyone have anything they're willing to share?
 
If not, I'm going to play around and create something and will share.  It won't be anything glamorous.

That's a good idea. I've been lazy and just writing things down but this would be much better.

I made an excel file along with my note book following the plants from germination to their position on the garden (with charts).. and then I stopped using it because I got enough work "on the field"... forgot about the paperwork at that point

i was designing this but seems like people enjoy the interaction from a grow log post here on thp instead of just logging it in a database.

Calendar. Sometimes I use a spreadsheet if I'm trying to track something specific.
